Registration Days and Hours

Monday - Friday

8:30 am to 11:30 am

Transfer release/check-out is to be presented at the time of enrollment

      

For your convenience, you are able to download registraion forms or pick them up at any time at Mar Vista Middle School.

 

Immunizations:

Original shot record will need to be provided to our school nurse prior to the enrollment.  Immunizations must be up to date.  For any questions regarding immunizations, contact Ms. Quillin at (619) 628-5108.

Special Education Students: Parent/Caregiver/Tutor/Foster Parent will need to meet with Ms. Sedor, School Psychologist, prior to the enrollment.  Ms. Sedor is available on Mondays, Wednesdays, and Fridays, must call for an appointment at (619) 628-5132 or (619) 691-5893.

 

Residency:

The State Department of Education now requires that each year all students must present AN ORIGINAL DOCUMENT (see below) as proof of residency in order to enroll in the Sweetwater Union High School District.

Proof of Residency may differ depending on each situation.  If you have any questions contact Mrs. Reyes at (619) 628-5120.

JOINT RESIDENCY (when your family lives with another family and uses their address), see the Joint Residency Checklist Form for a
list of the documents you will need to provide to us as your proof of residency.  For any questions contact Mrs. Reyes at (619) 628-5120.

CAREGIVER/TUTOR/FOSTER PARENTS, additional documentation is required.  Contact Mrs. Reyes at (619) 628-5107.

 

ONLY the following documents are accepted as proof of residency.

PROVIDE ONE (ORIGINAL) OF THE FOLLOWING DOCUMENTS DATED WITHIN 60 DAYS:

  • Utility bill: Gas & Electric, Water, Cable TV, Trash or Sewer
  • Lease Agreement/Rental Contract with Landlords’ name, address and telephone number and current rent receipt.                         
    **NO GENERIC RENTAL CONTRACTS ARE ACCEPTED**
  • Letter on apartment complex or mobile home park letterhead, signed by the landlord, stating that parent/guardian/caregiver lives there.
  • Closing Escrow papers, mortgage book or statement, or homeowner’s association fees statement.
  • Residence insurance statement; Verification of Social Services
